How Do We Predict an Outcome That Is Either Yes or No?

Suppose we record whether a component failed, along with the conditions under which it operated. We would like to know how those conditions relate to the chance of failure. A straight-line prediction can rise above certainty or fall below impossibility, so it cannot serve as a probability without further structure.
